    Student Info
  • Enter First name
  • Enter Middle name
  • Enter Last name
  • Choose Suffix from Drop Down List
  • Choose Grade Level from Drop Down List
  • Enter Arizona Student ID which will be assigned by the Arizona Department
  • Enter School Student ID which will be assigned by the Local School.
  • Choose Gender from Radiobuttonlist
  • Enter Birth Date or Choose date from the Calendar Icon
  • Choose Ethinicity from Drop Down List
  • Enter Zip code
  • Choose Country from Drop Down List
  • Enter the Entry date or Choose date from the Calendar Icon on which the student receives the Instructional services in the district.
  • Choose the Entry type
  • Enter Exit Date or Choose Date from the Calendar Icon will be the
    The date of an Individual last attendance in school
    The day on which the individual graduated
    The date on which it becomes known officially that an individual left the school
  • Choose the Exit type from Drop Down List if Exit date is entered
  • Enter the Notes (if any)
  • Finally Click the Submit button to save the Student Info
  • Click Cancel button to clear existing student details or enter the new student
