which is the assigned school code in Arizona education directory.
Enter the
Minutes per week
Choose the
Gifted Status
from the DropDownList. If Active is Chosen,
Enter the
Gifted Exit Date
or Select date from Calendar Icon
Choose the
Gifted Exit Reason
Enter the
Gifted Entry Date
or Select date from Calendar Icon
Choose the
Sped Environment
Choose
Yes or No for Academically Gifted
from Radio Button List

Yes or No for Creatively Gifted
